NEW STUDENT REGISTRATION
Please download a new student registration packet and have it completed before arrival with necessary documentation:
1) Copy of a current physical exam dated 9/01/11 or after
2) List of all childhood immunizations that are up to date
3) Name, address and fax number of the school your child is coming from
4) Copy of a transcript or report card
5) Copy of birth certificate
6) Proof of residency
